We’ll explore the nuances of these supplements, explain how they are generally tolerated in our practice, and offer practical tips on selecting the right ones for your unique needs.

Whether you’re just beginning your CIRS journey or looking to optimize your current regimen, we’re here to support you with the tools necessary for effective and personalized healing.

What Is CIRS?

Chronic Inflammatory Response Syndrome (CIRS) is a complex condition impacting multiple body systems with diverse symptoms. Triggered by significant inflammation from an overactive immune response, CIRS is acknowledged as a valid medical condition. Accurate diagnosis of CIRS involves comprehensive lab tests and a detailed examination of the patient’s medical history.

Due to its varied symptoms, CIRS is frequently misdiagnosed or inadequately treated. Unlike common immune disorders characterized by abnormal adaptive immune responses, CIRS cannot be identified through standard autoimmune blood tests. Consequently, a specialized and thorough diagnostic approach is crucial for the proper detection and management of this condition.

what is cirs

What causes the immune system to become hyperactive and lead to widespread inflammation in CIRS? The main culprits are biotoxins, which are naturally occurring toxic substances. These biotoxins, which trigger and worsen CIRS, come from several sources, including:

  • Water-Damaged Environments: Approximately 80% of CIRS cases are associated with exposure to air in water-damaged buildings. These environments contain mold mycotoxins, bacteria, and other chemical irritants that can provoke inflammatory responses in susceptible individuals. Both living and dead mold particles significantly contribute to inflammation in the body and brain.

  • Insect Bites: Bites from certain insects, such as ticks and spiders, can lead to chronic conditions such as Lyme disease. Pathogens such as Borrelia burgdorferi and Babesia microti, as well as venom from recluse spider bites, can cause long-term health problems.

  • Seafood Consumption: Consuming tropical reef fish contaminated with ciguatera toxin, linked to Pfiesteria and harmful algae blooms such as cyanobacteria (blue-green algae), can cause biotoxin-related illnesses. This toxin accumulates in larger reef fish that eat smaller fish containing dinoflagellates.

  • Contact With Contaminated Water: Exposure to or inhalation of water contaminated with toxic algae blooms, including Pfiesteria and cyanobacteria, can trigger the inflammatory responses typical of CIRS.

  • Other Biotoxin Sources: Additional triggers for CIRS include exposure to certain vaccines, viruses, volatile organic compounds (VOCs), endotoxins, and actinomycetes. These factors can all contribute to the onset and progression of the syndrome.

cirs water damaged building toxins

Biotoxins are microscopic molecules that can penetrate cell membranes, making them difficult to detect with standard blood tests. These toxins primarily enter the body through inhalation but can also be introduced via ingestion, insect bites, and direct contact with contaminated water.

Not everyone exposed to common biotoxins, such as mold, will develop CIRS. Genetic predispositions and life stressors play significant roles in susceptibility to biotoxin-related conditions. Factors such as severe illnesses, surgeries, high fevers, pregnancies, substantial biotoxin exposure, and other traumatic events can trigger a cytokine storm. This may activate the CIRS gene in genetically susceptible individuals, particularly those with HLA-DR genes, indicating that CIRS can develop at any point in life. It’s essential to understand that the absence of symptoms does not equate to immunity, and genetic testing can offer valuable insights for prevention.

While biotoxins might cause immediate illness, most people’s immune systems can identify and eliminate these toxins effectively. However, individuals genetically predisposed to CIRS have immune systems that fail to recognize and clear these toxins, allowing them to persist in the body. This leads to a chronic, severe immune reaction and widespread inflammation, resulting in various symptoms and long-term health issues.

What Is the Shoemaker Protocol?

cirs treatment shoemaker protocol

The Shoemaker Protocol is a thorough, evidence-based treatment plan developed by Dr. Ritchie Shoemaker to address CIRS and other biotoxin-related illnesses. It is the only peer-reviewed, clinically replicable treatment for biotoxin illnesses, including mold illness, grounded in rigorous scientific research and clinical trials, ensuring its effectiveness and reliability.

Beyond removing exposure, the Shoemaker Protocol includes a structured series of steps involving specific medications and therapies to reduce inflammation, correct hormonal imbalances, and restore proper immune function. Each step is designed to address various aspects of CIRS, ensuring a holistic approach to treatment. A combination of both prescription medications and natural supplements are used throughout the Protocol.

Patients utilizing this protocol can achieve significant health improvements, often leading to complete remission of symptoms.

The Importance of Personalized Supplements for CIRS Treatment

Creating a personalized supplement protocol is essential for effectively treating CIRS. This condition impacts multiple physiological systems, and the immune dysregulation and subsequent downstream effects can vary significantly from person to person. Due to the complex nature of CIRS, there is no one-size-fits-all approach.

Each individual’s unique biochemistry and health status necessitate a custom-tailored treatment protocol to address specific deficiencies, imbalances, and sensitivities. Personalized supplement protocols are designed to provide targeted support that aligns with the patient’s specific needs, enhancing the efficacy of the overall treatment plan.

Immune dysfunction caused by CIRS often leads to co-infections and comorbidities, adding layers of complexity to the treatment process. These additional health challenges require careful consideration when selecting supplements, as well as determining the appropriate timing within the protocol and adjusting dosing to minimize Herxheimer reactions (herxing).

Pro-Tip: Download our free Herxheimer Guide for all the best tips for minimizing herxing.

Co-infections such as Lyme disease, viral infections, gut imbalances, and other chronic conditions often complicate the clinical picture, necessitating a multifaceted approach. Supplements must be chosen not only for their primary benefits but also for their interactions with other treatments and their ability to support the immune system without exacerbating symptoms.

Working with a trusted CIRS practitioner is essential to navigate these complexities and determine the ideal supplements for your personalized healing journey. Experienced practitioners can assess individual health markers, identify specific needs, and create a tailored protocol that addresses the root causes of CIRS and its associated conditions.

They can also provide guidance on adjusting dosages, managing potential herxing, and ensuring that supplements are integrated effectively into the broader treatment plan. This personalized approach ensures that each patient receives the most appropriate and effective support for a successful recovery from CIRS.

That being said, we’ve prepared a general supplement guide below for reference.

General CIRS Supplement Guide

In this guide, we will share general CIRS supplement recommendations that have proven effective for some or many of our patients. At our practice, we work with some of the sickest individuals, including those who struggle to tolerate even the most commonly well-tolerated supplements.

If you find yourself in this situation, know that you are not alone. It’s important to consider mind-body work, gut healing protocols, and histamine reduction as potential strategies to improve your tolerance threshold. By addressing these underlying issues, oftentimes you can enhance your ability to tolerate and benefit from the recommended supplements on your healing journey.

Remember, always speak with your trusted CIRS practitioner regarding starting any new supplements.

Pro-Tip: We recommend purchasing your supplements from trusted sources such as FullScript as they temperature-control their warehouses to maintain the integrity and quality of your supplements. Amazon does not temperature-control its warehouses and works with multiple third-party sources.

Foundational Supports

Here are foundational supports for CIRS treatment, focusing on essential minerals that help establish a strong baseline of health. These foundational supplements are important for overall well-being and play a vital role in supporting the body’s natural healing processes.

Ensuring adequate levels of these nutrients can provide the necessary support to enhance immune function, reduce inflammation, and promote recovery. By starting with these basic supports, you can build a solid foundation for more targeted interventions in your CIRS treatment plan.

Electrolytes and Minerals

Sole water is the most tolerated electrolyte option, ideal for individuals with many supplement sensitivities. It is also the most cost-effective option as it only requires high-quality mineral salt and filtered water to make at home.

Overall Pick
  • Quicksilver Scientific Original Quinton Isotonic: Some of our patients have to start with a few drops a day and slowly work their way up. It comes in individual glass ampoules designed for daily dosing with up to 78 bioavailable minerals. For a comprehensive electrolyte mineral formula, this option is generally the second-best tolerated compared to sole water. This is our favorite electrolyte option from this list based on tolerability, mineral inclusion, and glass packaging.

Additional Electrolyte Options
  • Trace Minerals ConcenTrace Trace Mineral Drops: This is a lower-sodium electrolyte option that may require additional salt or sole water depending on your needs. It contains over 72 bioavailable minerals. We also recommend starting low and slow with this option and working your way up.
  • BodyBio E-Lyte: The concentrated electrolyte formula contains only pure sodium, potassium, and magnesium, making it ideal for individuals who may be sensitive to consuming other minerals. BodyBio E-Lyte mimics your own body fluids and is very well tolerated. Start with one capful per day in a glass of water and increase as needed.  Many low-carb dieters will require at least 3-4 capfuls daily.
  • Biotics Research Multi-Mins: The iron and copper-free multi-mineral formula comes in tablets for individuals who prefer taking their electrolytes in pill form. This broad-spectrum option is generally tolerated better by individuals without many supplement sensitivities.

Iodine

Iodine is crucial for the body’s thyroid function and overall metabolic health, playing a key role in hormone production and detoxification processes. However, not everyone can tolerate iodine early in their CIRS treatment due to potential sensitivities or detox reactions. For some individuals, it may be necessary to introduce iodine later in the treatment protocol once their system is more stable and better able to handle it.

  • J. CROW’S Lugol’s Solution 2% Iodine: If you’d like to start supplementing iodine, you can download our free guide here. Remember to always start with the lowest dose and be mindful that some people don’t tolerate this until later on in treatment.

Optional Pre-CIRS Treatment Binders

Some individuals may start with optional binders before beginning prescription binders such as Cholestyramine (CSM) and Welchol. Practitioners might require this step for ultra-sensitive patients to help their bodies adjust gradually. However, this approach may not be suitable for everyone, as it can be influenced by the presence of other co-infections. Each patient’s unique health profile must be carefully considered to determine the best course of action.

Overall Pick

Additional Pre-Treatment Binder Options

  • Premier Research Labs Medi-Clay-FX: The single-ingredient bentonite clay is another option that may be considered for pre-CIRS treatment binding.
  • Metabolic Code MycoBind: Supporting research shows that the active ingredients of okra extract and organic beet juice in this natural binder have a reduced binding affinity to negatively charged biotoxins. MycoBind is about 16%-20% as effective as CSM. Unlike other natural binders that have the opposite charge and therefore aren’t capable of binding the biotoxins responsible for CIRS, MycoBind may also be used for individuals who don’t tolerate either CSM or Welchol. However, individuals with oxalate sensitivities should not take this product. Oxalate issues can be somewhat common with CIRS as certain fungi can produce oxalates in the body post-colonization.
  • CellCore BioToxin Binder: With various long-, medium-, and short-chain carbons, this broad-spectrum binder targets a variety of different toxins. This is generally tolerated by individuals without many supplement sensitivities. Individuals with small intestinal fungal overgrowth (SIFO) may not tolerate this due to the citric acid. Individuals with histamine intolerance or Mast Cell Activation Syndrome (MCAS) may also not tolerate this.

Pro-Tip: For a more in-depth, nuanced understanding of binders for CIRS treatment, you can read our prescription binders vs. natural binders resource here. It is important with all these natural binders to take them away from food, medications, and supplements (45 minutes prior to meals and two hours after meals).

Lipid Replacement

Lipid replacement therapy is essential for CIRS patients, as it helps restore and stabilize cell membrane integrity. This therapy is a critical step in preparing patients to better tolerate prescription binders CSM and Welchol, which are the second step in the Shoemaker Protocol. By improving cell membrane function, lipid replacement therapy enhances the effectiveness of these binders, facilitating the removal of biotoxins and supporting the overall healing process. We have thoroughly researched the lipid replacements we recommend. Lipid replacement is a category of the supplement industry that is very compromised if you don’t know the methods they use to extract and preserve fatty acids from marine-derived sources.

Overall Pick

  • Omaprem SPM Resolvins Fatty AcidsThis specialized pro-resolving mediator (SPM) formula is the only product that contains a total of 30 essential fatty acids and is backed by over 25 years of multiple clinical studies. Omaprem and other SPMs are generally better tolerated by those with histamine issues compared to other fish oils but aren’t tolerated by everyone with histamine intolerance and MCAS. This is our favorite lipid replacement option from this list due to its effectiveness, clinical backing, and general tolerability. We usually recommend taking a mix of Omaprem with another lipid replacement supplement to lower costs.

Additional Lipid Replacement Therapy Options

  • Microbiome Labs Mega MarineThe full-spectrum fish oil contains a ratio of omega-3s specifically targeted for gut support. This option usually isn’t suitable for those with histamine concerns.
  • Biotics Research Biomega-500: With minimal filler ingredients, this is another great fish oil option to consider for lipid replacement therapy. However, Biomega-500 may not be suitable for those with histamine concerns as well.
  • Metagenics SPM ActiveSPM Active is another clinically-backed SPM option to explore. It is tolerated by some individuals with histamine intolerance and MCAS.
  • BodyBio PC LiquidSome individuals do well with this as it mimics the cell’s outer layer fat composition, particularly if you are low in omega-6 fatty acids. For those concerned about seed oils, certain plant-based oils can play a therapeutic role when the body is imbalanced. This option generally isn’t suitable for those with histamine concerns.

Pro-Tip: If you are unable to tolerate any of these options due to histamine intolerance or MCAS, there are other ways to complete lipid replacement therapy. Always work with your trusted CIRS practitioner for personalized solutions for you.

Inflammation Support

Inflammation support is crucial for managing CIRS due to the inherently inflammatory nature of the condition. Incorporating natural anti-inflammatory strategies can significantly help reduce symptoms and improve overall well-being. Utilizing anti-inflammatory supplements along with anti-inflammatory diets such as Carnivore can aid in managing inflammation and supporting the body’s healing process.

Overall Pick

  • Omaprem SPM Resolvins Fatty AcidsClinically proven for inflammation reduction, Omaprem also offers significant inflammation management in addition to lipid replacement therapy. This is an ideal option for individuals who tolerate SPMs in order to reduce the number of supplements they’re taking. This is our favorite inflammation support option due to its effectiveness.
  • Pure Encapsulations Quercetin: Quercetin is a flavonoid and antioxidant that offers anti-inflammatory and histamine-reducing properties. This single-ingredient option is generally well-tolerated. This is our favorite inflammation support option for individuals with histamine concerns who are unable to take Omaprem.

Additional Inflammation Management Options

Gut Support

Gut support is vital for CIRS patients, as gut health is often compromised by the biotoxin pathway. Biotoxins can disrupt gut function and lead to common co-infections, further exacerbating health issues. Ensuring proper gut support helps maintain nutrient absorption from food, which is essential for overall healing. Incorporating probiotics, digestive enzymes, and gut-healing supplements can strengthen gut integrity and support the body’s recovery process.

Overall Pick

  • NwJ Gut Healing KitOur Gut Healing Kit combines four therapeutic nutritional supplements for supporting optimal gut health. It provides stomach acid and other powerful digestive enzymes to support the breakdown and absorption of foods, fat metabolism support for healthy bile flow needed for CIRS binders, practitioner-grade spore probiotics for healthy gut barrier function, and immunoglobulins for additional toxin removal. This is our favorite combination of gut supports for comprehensive gut healing and digestion.

Additional Gut Support Option

  • Microbiome Labs RestorFloraRestorFlora contains powerful beneficial probiotic yeast called Saccharomyces boulardii (S. boulardii) that offers therapeutic value for gut infections, antibiotic use, and Candida. Some individuals with CIRS develop sensitivities to S. boulardii and may not tolerate this.

Histamine and MCAS Support

Histamine support is often necessary for CIRS treatment, as histamine intolerance and MCAS can be common with this condition. Managing histamine levels is crucial to prevent symptoms from flaring up and to ensure better tolerance of the various supplements and medications required during treatment. By incorporating histamine-lowering strategies and supports, individuals can effectively manage their “histamine bucket,” improving their overall response to the comprehensive CIRS treatment protocol.

Overall Pick

Additional Histamine Support Options

  • Apex Energetics Histo-XThe herbal anti-histamine blend offers histamine support for both food and environmental factors.
  • Pure Encapsulations Perilla ExtractPerilla extract offers anti-histamine and anti-inflammatory benefits, particularly for asthma and allergies. Perilla is generally well-tolerated but a small population can be allergic.
  • OmneDiem Histamine Digest PureMAXDiamine oxidase (DAO) is the primary enzyme responsible for breaking down dietary histamine. Individuals with pork sensitivities, alpha-gal syndrome, and other certain supplement sensitivities may not tolerate this option. This works best taken 10 minutes prior to each meal.
  • Biotics Research HistoPlex-ABThis contains a blend of herbal extracts shown to possess immunomodulating properties. It is generally tolerated by those without many supplement sensitivities and who don’t have more severe histamine issues.

Detox Support

Poor detoxification is a significant factor in CIRS, as the body’s ability to eliminate biotoxins is compromised. Additional detox supports can be beneficial in enhancing the body’s natural detoxification processes. However, it’s important to recognize that not everyone can tolerate detox supplements, as they may place additional strain on already stressed drainage pathways. Careful consideration and personalized guidance from your trusted CIRS practitioner are essential.

Overall Pick

  • Biotics Research Mo-Zyme ForteMolybdenum is a mineral that helps break down toxic substances in the body and is a necessary cofactor for detox processes. Individuals with low copper should work with a trusted practitioner when supplementing molybdenum as the two antagonize each other and can reduce the absorption and utilization of copper. This is our favorite detox support in terms of general tolerability and multi-use purpose.

Additional Detox Support Options

  • Biotics Research NACN-acetyl cysteine (NAC) is an antioxidant, a precursor to glutathione, and supports the body’s detox process. Individuals with sulfur sensitivities and histamine concerns may not tolerate NAC.
  • Body Bio Liposomal GlutathioneGlutathione, commonly referred to as the master antioxidant, offers comprehensive detox support throughout the body. Individuals with sulfur sensitivities may not tolerate glutathione.
  • Biotics Research GSH-PlusGSH-Plus contains NAC, L-Glutathione, and glycine for optimal detox support. Individuals with sulfur sensitivities and histamine concerns may not tolerate this option.

Pro-Tip: The three additional detox support options above may have a sulfur smell which is normal and safe. Some individuals aren’t able to tolerate any of these detox supports until later on in treatment. Castor oil packs and topical magnesium can be gentler detox options to try. Always start low and slow as some of our ultra-sensitive patients can experience Herxing.

Sleep Support

One common symptom of CIRS is poor sleep, which often necessitates temporary support throughout treatment. Prioritizing natural sleep aids, such as melatonin, magnesium, or herbal remedies, is important before considering prescription options due to the potential risks associated with sleep medications. Natural supports can help improve sleep quality with fewer side effects, supporting overall recovery and well-being.

Overall Pick

  • Biotics Research Bio-AshwagandhaAshwagandha has been shown to offer improvements in sleep and anxiety. This is our favorite sleep support option in terms of general tolerability, multi-use purposes, and effectiveness.

Additional Sleep Support Options

  • Pacific BioLogic Soothing SleepThe herbal blend works synergistically to calm your mind, promote relaxation, and support deep, restful sleep. Individuals who are sensitive to many supplements may not tolerate this multi-ingredient formula.
  • Alpha-Theta Ultra PMAlpha-Theta supplies key nutrients recognized to induce calming brainwave activity and to promote the onset and duration of high-quality sleep. Individuals with severe histamine issues may not tolerate the gelatin used for these capsules.
  • Biotics Research Mult-MinsThe iron and copper-free mineral formula recommended for electrolytes can also support sleep with its magnesium content. Individuals who are sensitive to many supplements may not tolerate this multi-ingredient formula.
  • Seeking Health Vitamin D3 + K2Vitamin D has both a direct and indirect role in sleep regulation. We generally only recommend supplementing with vitamin D3 and K2 if labs warrant it.

Stress Management

Stress management is crucial for CIRS patients, as chronic stress can worsen the already heightened inflammatory response. It’s essential to develop a comprehensive stress management toolbox, incorporating techniques such as mindfulness, exercise, and relaxation practices. Relying solely on supplements for stress reduction is never recommended, as a multi-faceted approach is more effective in promoting overall well-being and supporting the healing process.

Overall Pick

Additional Stress Management Option

Pro-Tip: Are you looking for comprehensive stress management tools for your CIRS healing journey and everyday life? We’ve developed an exclusive Mind Body Program with our in-house clinical therapist that’s ideal for helping our chronically ill with root-cause healing.

Energy Support

Fatigue is another common symptom for CIRS patients, often requiring additional energy support. This persistent tiredness can significantly impact daily life and hinder the healing process. Incorporating energy-boosting strategies, such as balanced nutrition, appropriate exercise, and targeted supplements, can help alleviate fatigue and enhance overall vitality.

Overall Pick

Additional Energy Support Options

Methylation Support

Individuals with the methylenetetrahydrofolate reductase (MTHFR) mutation often require additional methylation support, which is crucial during CIRS treatment. Proper methylation is essential for effective detoxification and overall healing.

Overall Pick

  • Biotics Research Methylfolate PlusMethylfolate Plus contains folate in its active form along with glucosamine salt and methylcobalamin for comprehensive methylation support.

Brain Support

Brain supports can be incredibly helpful during CIRS treatment, as the condition often leads to uncontrollable inflammation, resulting in a “brain on fire” sensation. This inflammation can cause cognitive issues, brain fog, and neurological symptoms. Incorporating brain-supporting supplements and strategies can provide neurological benefits and help reduce inflammation, supporting overall cognitive function and mental clarity during the healing process.

Overall Pick

  • Omaprem SPM Resolvin Fatty AcidsAs our favorite lipid replacement therapy option that also offers anti-inflammatory benefits and brain support, Omaprem is a great multi-purpose option to explore. Some individuals with histamine concerns may not tolerate Omaprem. This is our favorite brain support option in terms of general tolerability, effectiveness, and multi-use purpose.

Additional Brain Support Options

  • Biotics Research Cytozyme-PT/HPTCytozyme offers glandular support for overall brain health. Some individuals with severe histamine issues may not be able to tolerate this.
  • Klaire Labs SFI Health Benfotiamine: Thiamine is an essential nutrient that also plays an important role in the brain and helps support cognitive function. Individuals with sulfur sensitivities may not tolerate this.
  • BodyBio PC LiquidSome individuals find therapeutic value in brain support as it may improve brain function. This option generally isn’t suitable for those with histamine concerns.
  • Synapsin Spray (Rx Only): The compounded prescription nasal spray contains Panax ginseng and nicotinamide riboside to help reduce neural inflammation and offer neuroprotective properties.

Anti-Viral Support

Anti-viral supports can be beneficial during CIRS treatment, as viral infections often coexist with biotoxin-related illnesses, exacerbating symptoms and complicating recovery. Incorporating anti-viral supplements and therapies can help individuals reduce the viral load, supporting the immune system’s ability to manage and clear infections.

Overall Pick

  • Gaia Pro Olive Leaf 1360Olive leaf contains antioxidant properties to help maintain the body’s natural immune defenses. It also contains anti-viral, antimicrobial, and antifungal properties. Individuals with known co-infections should proceed with caution and work with their trusted practitioner if interested in taking an anti-viral during CIRS treatment as this can cause potential Herxing in these cases.

The Importance of Diet During CIRS Treatment

Carnivore Diet Food Pyramid

We highly recommend eating a carnivore diet or a heavily meat-based diet during CIRS treatment as it offers comprehensive support across all the various categories above, particularly with inflammation management. We’ve found that our meat-based patients often require fewer supplements during CIRS treatment.

You may have heard that ketogenic and carnivore diets aren’t suitable for everyone with CIRS. Stay tuned as we will release an article that will examine all the various nuances behind this and why we believe Carnivore is still ideal for healing.
